Policies

Practice policies are designed to help Foundations run smoothly, we appreciate your consideration and thank you for understanding.

Payment

Payment is expected at the time of service.  All intake sessions will be confirmed when all intake documents have been completed and a credit card is on file in the client portal.  Cash and credit card are acceptable forms of payment.

Cancellation Policy

A 24 hour notice is required for all canceled appointments.  Intake sessions will be confirmed when all portal documents are completed and a credit card is on file in the client portal.  Less than a 24 hour notice will result in a charge for the full session fee.  A missed appointment with no cancellation will incur a charge for the full session.  Any charge backs from credit cards will incur a $50 fee in addition to any late fees.

New Clients

If you are a new client, you must complete all documents and upload a form of payment to the portal within 72 hours. After 72 hours, your standing appointment time slot will be cancelled and your request for services closed.

Verifying Identity

A copy of your driver’s license or other government issued identification will be used to verify identity for financial payment when necessary and to validate your identity for records requests.

Conservatorship, Guardianship, or Parental Rights

We are required by the State of Texas to collect, maintain, and review a current copy of any court ordered conservatorship, guardianship, or parental rights regarding a minor client.  Clients must upload a copy of such documents to the appropriate portal.

Social Media

We maintain professional relationships with our clients that maintains their privacy and preserves the professional role of each therapist.  We do not engage in social media contact with current or former clients.
